These benefits are applicable only at Darling Dentistry, located at 115 Old Boerne Road, Suite E, Bulverde, Texas 78163 and only applicable to services provided by appointment, during business hours, by Dr. Kelsey B.P. Garza, General Dentist, and her employees.

This plan does not apply to any services provided by a specialist, whether or not these services are completed at Darling Dentistry.

The reservation fee is non-refundable under any circumstances, including voluntary or involuntary dismissal from the practice. The reservation fee is normally $249, and fee and benefits are subject to review and change. You will be notified of any such change prior to any renewal.

This is not an insurance plan. This offer cannot be used in conjunction with any other offers or with discount plans such as dental insurance, and 15% savings will not apply when paying through financing, such as Cherry or Care Credit.

You will not receive a membership card. The effective term period will be on file with our office, and this information will be readily available to you.

The term of participation is for one year and begins on the first date of service at our office following purchase OR December 31, 2024 (whichever comes first) The last day of benefits is one day prior of the following year (Ex: Sign-Up Date: March 2, 2022 Expiration Date: March 1, 2023).

Procedures performed will be in accordance with the doctor’s professional judgment and treatment plan options provided.

Payment for services is due in full no later than when services are rendered. Some appointments may require pre-payment to reserve. If a patient leaves without paying for services provided in full for any reason, this is considered theft of services. If we have still not received payment in full within 24 hours of the appointment, the patient will become ineligible for participation in the savings club, and savings will NOT be valid for that appointment or any future appointment.

It is the sole responsibility of the participating patient to maximize benefits by arranging the appropriate appointments within the 12 month period.

To continue receiving participant benefits, the reservation fee will be due on the same date each year.

Patients with two no-show appointments within a 12 month period may become ineligible for participation in the savings club and will receive no refund for the reservation fee.

Patients with three short notice cancellations or combined no-show/short notice cancellations within a 12 month period may become ineligible for participation in the savings club and will receive no refund for the reservation fee.

Patients with excessive short notice cancellations or no-show appointments may be subject to dismissal from the savings club and from the practice.

No-show appointments are defined as providing less than 1 hour’s notice or no notice to cancel or reschedule an appointment.

Short notice cancellations are defined as canceled/rescheduled appointments with less than 48 hours notice.
